Langley Corner Surgery

Online patient request system.

This is a great way for you to contact our practice for medical appointments or administration requests without the need to call the surgery. The process is very simple and will only take a couple of minutes to complete.  

You will be asked to complete an online form via the green New Patient Request Form button above or you can save the link to your device. 

Forms can be submitted daily from 8am Monday to Friday and will remain open until the surgery has reached its capacity for that day or 3pm.

We really think it will be easier for most people than waiting on hold to speak to a receptionist. If you do not have access to the internet or find it difficult to use a computer or smartphone you will still be able to call our reception team who will support you with the form.

Please be considerate whilst we make these changes, start the new process and resolve any teething problems
